*PLEASE NOTE: If you have had a procedure, you may receive up to 4 separate statements reflecting charges for services provided by your physician, the facility (endoscopy center), the pathology laboratory and the anesthesiologist.

Due to high call volumes patients may experience long wait times. If you need to schedule a routine appointment, please use the "Schedule a visit" button in the top left. Established patients may use their patient portal for any non-scheduling inquiries, such as prescription or records requests.
